Shop Break

The Shop Break forms are available in the cafeteria and the main office.

  • Please be sure to write your shop name on the bottom of the order form. (If we have an envelope with money and no name, the right product doesn’t get delivered.
  • Students need to put the proper name and ID number so that it is legible on the shop break form.
  • Students needing change need to state this beside their name on the form. (All change is placed in the return envelope and initialed by the cafeteria.)

Submit your completed form or letter to USDA by mail to U.S. Department of Agriculture, Office of the Assistant Secretary for Civil Rights, 1400 Independence Avenue, SW Washington D.C. 20250-9410, by fax at (202)690-7442, or email it to program.intake@usda.gov.Persons with disabilities who require alternative means of communication for program information (e.g. Braille, large print, audiotape, American Sign Language, etc.), should contact the Agency (State or local) where they applied for benefits. Individuals who are deaf, hard of hearing or have speech disabilities may contact USDA through the Federal Relay Service at (800) 877-8339. Additionally, program information may be made available in languages other than English.
